Yeh, with the extension open at the bottom of your browser the best way is to right-click on any of the tools and hit 'Deploy x'. After that you should see a small notification pop up in the bottom right of your browser.

If you want to hide the toolbar but still play for a while, you can just click the 'P' icon in the bottom right corner and it'll dissappear. You'll still be playing the game and you'll see the occassional alert though - if you really want to take a break from PMOG then hit the 'eye' icon in the bottom left of the window. That'll pause the game and you'll be out of PMOG until you decide to switch it back on!

Woo - this has gotten complexificated all of a sudden ;-)

There are a couple of extensions that "break" PMOG - I guess were all abusing the global namespace commons. I'll let our engineers know about this one. Thanks, akraut. I knew we were allies for a reason. ;)

Just validating akraut's problem: I have to leave the toolbar open if I want any orange mission boxes or portals to show up as well. I've gotten used to it, though :)
